This year's conference took a transformative step by merging Shell's Rotterdam and London Focus Group to create a Europe-Africa (EU-AF) Focus Group into a single consolidated forum. The central theme, “Fail Safely or Fail Luckily,” challenged participants to rethink their approach to risk and human error. Instead of relying on luck to avoid accidents, the discussions focused on proactively designing systems and barriers to anticipate/prevent failure and safeguard lives.
From a compelling 'live theater acting' collision scenario to powerful crew testimonials, the sessions created an emotional and intellectual space to reflect on the realities of life at sea. Key takeaways included the vital role of Human Performance principles in day-to-day operations, the importance of embedding safety at the leadership level, and the necessity of cross-industry collaboration to ensure safer outcomes for all.
